Use Any Doctor, Hospital, Emergency Room or Medical Facility
Benefits are subject to a $100 deductible per accident per covered family member. Covered expenses means the reasonable and customary charges for a local professional ambulance service to or from a hospital and/or surgical center as well as the following reasonable and customary charges for treatment, services, and supplies provided or prescribed by a doctor:
Hospital Room and Board * Surgical Center care and treatment * Outpatient Hospital Emergency Room * Surgical Benefits * Doctor's Visit In-Hospital * Doctor's Visit Out-Patient * X-ray and Laboratory * Nursing Care * Physiotherapy * Ambulance * Medical Equipment Rental * Medical Services and Supplies (Blood, Blood Transfusions, Oxygen) * Prescription Drugs * Dental Treatment as a result of injury to sound, natural teeth.

Use Any Hospital
Benefits are payable for a maximum of 30 days, zero waiting period. If an accidental bodily injury results in the Insured's hospitalization within 45 days of the accident, the plan will pay a daily benefit for each day of hospitalization beginning the first day of hospital confinement for a maximum of 30 days. Such confinement must be prescribed by a Physician and as a regular bed patient.

Benefit Amount Accidental Death: $16,000 (Principal Sum)
If within one year from the date of an accident covered by this policy, injury from such accident results in the death of the covered person, the plan will pay 100% of the principal sum shown above.
Benefit Amount Dismemberment: $16,000 (Principal Sum)
The plan will pay the amount shown above for a covered person who suffers a dismemberment, paralysis, or loss of sight, speech or hearing.

$4,000 Helicopter Emergency Transportation Benefit
|
The emergency helicopter rescue insurance provides up to $4,000 payment to helicopter service provider for each occurrence when a member suffers from a Certified Injury that requires Emergency Medical Transportation by a helicopter in accordance with local EMS protocols. The policy includes $1,000 scheduled airline AD&D, and this amount is in excess of all other valid collectible insurance.
